Agency Certification Forms

In order to become a CARE Scholar an Agency Certification Form (AC) has to be filled out by the students Gain/Cal-Works worker. This form is used by C.A.R.E. to verify that the student is single, head of house hold, is receiving cash aid, and has a child. The Agency Certification form must be turned in by the deadline each semester in order to be eligible. Due to the current COVID-19 Pandemic, we are accepting Verification of Benefits Forms instead of the Agency Certification. This will take effect for the Fall 2020-Spring 2021 Academic Year.
